Support Services
REACH's Support Services program helps people live as indpendently as possible by providing support in the home, in the community or on the job.
REACH's Support Services help people live as independently as possible by providing support in the home or community.
Our services include:
Assisted Living Homes: REACH provides full-time care for adults in a group setting. REACH has three state-licensed assisted living homes that have from two to four residents.
Day Habilitation: REACH staff provide support so that people can be as independent as possible in the community.
Family Habilitation: Children or adults live with a qualified adult or family full-time, out of their own home. REACH works with the State of Alaska to license Family Habilitation for consumers. This type of care does not require the natural family to give up custody or parental rights.
In-Home Supports: REACH staff help people at home with the normal daily routines of their lives. For example, a service provider may assist with the routine of getting ready to go to school. The amount of time staff spend with a person depends on the individual's needs.
Shared Care: Two families, one of which is usually the natural family, share the care of an individual with a disability. Over the course of twelve months the person with the disability spends approximately 50 percent of the time with each family.
Supported Living: REACH staff provide support to adults living independently in the community. The amount of support varies depending on individual needs. For example, someone may need help with weekly grocery shopping, while another person may require assistance with self-help skills on a daily basis.
We also offer staff assistance with specialized equipment and environmental modifications.
Care Coordination: A REACH care coordinator helps clients and their families access services which match their needs and choices and then monitors those services for quality of delivery.
Supported Employment: People are provided whatever assistance is needed to find and maintain employment. This support may include completing applications, interviewing for a job, on-the-job training, job coaching or accommodations needed for someone to work more independently.
