REACH Support Services

  • Case Management - case managers work closely with clients to ensure they have the support and resources needed to achieve their goals.
  • Day Habilitation – assistance with self-help, socialization and adaptive skills in the community such as accessing community activities indoors and outdoors, like swimming, shopping, art studio classes, or getting ready to go to school.
  • Shared Care – two families, one of which is usually the natural family, share the care of an individual with a disability.  Over the course of twelve months the person with the disability spends approximately 50 percent of the time with each family.
  • In-Home Supports – REACH Staff help children at home with the daily routines of their lives like cooking, dressing or cleaning.
  • Supported Living – for people age 18 and over living as independently as possible in their own home.  Staff assists with daily routines (cooking, meal planning, hygiene, housekeeping, etc.) and works with employers, family members, medical providers, transportation providers, social workers, etc. to ensure all daily living needs are being met.
  • Family Habilitation – children or adults live with a qualified adult or family full-time, out of their own home. This type of care does not require the natural family to give up custody of parental rights (REACH works with the State of Alaska to license Family Habilitation Homes for clients)

Here are the words of some people with disabilities:

“Do not try to fix me because I am not broken. Support me. I can make my contribution to the community in my own way”

“Do not see me as an obligation – I am your fellow citizen. See me as your neighbor. Remember that none of us are self-sufficient.”
